To My Founding Readers: A Promise—and What Comes Next
Originally published on Substack.
When I began Beyond the White Coat, I hoped to create a place where medicine could meet meaning—where we could talk not only about adding years to life, but also about adding life to those years.
Today, more than 11,000 of you read these letters. You have shared them with family, replied with your own stories, challenged me with thoughtful questions, and reminded me that health is never only about a number on a chart. It is about the people we love, the choices we make, the faith that carries us, and the life we are still becoming.
I am deeply grateful. This publication has grown because you made room for it in your lives.
THE LONGEVITY CIRCLE—AND A PROMISE THAT COMES FIRST
On Wednesday, October 7, 2026, I am introducing The Longevity Circle, an optional paid membership for readers who want to go deeper and help extend this work into the world.
The Founding Reader promise: If you are already a subscriber when The Longevity Circle launches, you are a Founding Reader. You will keep full access to everything I publish on Beyond the White Coat—forever, free. Payment will remain entirely optional for you. Nothing is being taken away.
I want that to be unmistakable. You helped build this community. I will not thank you by placing a locked door where there was once an open one.
THE HEART OF BEYOND THE WHITE COAT STAYS FREE
For everyone—now and in the future—the weekly essays will remain free. I will continue publishing two to three times each week on the subjects that brought us together:
-
Longevity—the science and daily practices that can help us live longer and live better.
-
Heart health—clear, practical guidance informed by decades of caring for patients.
-
Faith—the questions, rituals, and moments of gratitude that help us remain grounded.
-
The examined life—reflections on purpose, relationships, aging, loss, and what makes a life meaningful.
These essays are the foundation of Beyond the White Coat. They will remain open and free, because trustworthy health information and thoughtful conversation should not belong only to those who can pay.
THE CIRCLE ADDS DEPTH, ACCESS, AND CONVERSATION
The Longevity Circle is for readers who want a more focused layer of learning and connection—and for those who want their support to become a gift beyond these pages.
Membership will include:
-
Two in-depth deep dives each month on the questions that deserve more than a quick answer.
-
A monthly live Q&A where we can explore your questions together in real time.
-
48-hour early access to new essays before they are released widely.
-
An annual Longevity Report bringing together the most useful developments in heart health, prevention, aging, and well-being.
-
Paid subscriber discussion threads for more sustained, thoughtful conversation within the community.
For Founding Readers, every piece of writing I publish will remain available without charge. Joining the Circle is an optional way to participate in its additional experiences and support the charitable mission around this work. If the free essays are what serve you best, please keep reading them with my gratitude.
MEMBERSHIP IS SIMPLE
Beginning on Wednesday, October 7, 2026, new subscribers will be able to choose between the free tier and The Longevity Circle. Existing subscribers remain Founding Readers under the promise above.
$20 per month, or $180 per year.
For those who feel called to make a larger commitment, there is one additional way to stand behind the mission:
-
Founding Member — $1,800 per year: Everything included in The Longevity Circle; invitations to two intimate online roundtables each year; advance looks at major projects and new initiatives; optional recognition in the annual community thank-you.
There is no pressure and no countdown. Choose the level that feels right—or choose none at all. If you join, I will be honored to welcome you.
EVERY MEMBERSHIP DOLLAR WILL GO TO CHARITY
This is the part of the new chapter that means the most to me: every dollar raised through The Longevity Circle, Founding Member memberships will be given to Save a Child’s Heart — the nonprofit providing free lifesaving heart surgeries to children around the world (saveachildsheart.org).
I am not creating these memberships as another source of personal income. I want the generosity of this community to travel outward—to help people, strengthen lives, and turn thoughtful conversation into tangible good.
I have also chosen to keep Beyond the White Coat sponsor-independent. Health information is too important to be shaped by what an advertiser wants mentioned, avoided, or softened. Membership will never buy influence over what I write; it will support deeper participation here and charitable impact beyond these pages.
That trust is more valuable than any advertisement. I will do my best to earn it, one essay, one conversation, and one act of service at a time.
WHATEVER YOU CHOOSE, YOU BELONG HERE
If you become a member of The Longevity Circle—or choose to join as a Founding Member—thank you. Your gift will help carry the spirit of this work beyond the page through Save a Child’s Heart — the nonprofit providing free lifesaving heart surgeries to children around the world (saveachildsheart.org).
If you remain a free reader, thank you. Your attention, your trust, and the life experience you bring to this community are gifts in their own right.
The goal has not changed: to help one another live longer, live wiser, and pay closer attention to the life unfolding in front of us.
I am grateful that you are here—and especially grateful to welcome you as a Founding Reader.
Blessings,
Afshine Emrani, MD, FACC
